For each section, make the following selections:
SECTION I: IDENTIFICATION
Fill your contact information in the form. Please note:
Under “Firm/Business or Agency,” enter the name of your organization. If you are not a member of a business/agency, you can put “On behalf of self.”
Under “Title,” enter your title. If you do not have a title, you can put “NA.”
SECTION II: REPRESENTATION
Enter “Illinois voter” or “Illinois resident.”
SECTION III: POSITION
Select in the drop down menu “HFA 1”
Select “Opponent”.
SECTION IV: TESTIMONY
Select “Record Of Appearance Only.”
Select “I Agree to the ILGA Terms of Agreement.”
Click “Create Slip” on the right side.
