Request to Participate in our Annual Service Delivery Survey
I am writing to you to request your participation in a brief survey.
As
you may know we have been providing a kenneling service for 60 years.
Our future depends on meeting your needs and this survey aims to provide
us with valuable feedback on your recent pet care experience that we
will use to improve our services. The survey is very brief, it is just 10 questions and will only take about 5 minutes to complete.
I
have tried to add a fun element of surprise by adding a few quirky
elements. This said, please be assured that we value your advise.
Your participation in the survey is completely voluntary and all of your responses will be kept confidential.
