Under Training Offerings:

• Select OST Onsite Training from the drop down,

• Enter the Training ID number under Training ID # OST20230401-MIOH
  (make sure there are no spaces)

• Fill out your information

• Click register

• You will receive an email letting you know that our Training team has received your information.

• The Zoom link and Meeting handouts will be sent out the Monday or Tuesday the week of the scheduled training.

This multi-purpose form has 3 sections to complete:

1. About you, where you'll enter your name, email address and phone number

2. About this payment, providing:

   1. Payment Type, "Select Other Payment"

   2. Additional details, use the Training ID number from above

3. Pay here, where you'll enter your payment information.
