office at 707-527-7313. This will reduce the amount of time we have to
spend calling everybody.

To further clarify, you may either apply the visit fee to future care here or
get an immediate refund.

If you choose a refund we can either issue a check or refund to your credit
card. If you want a check, please be sure to let us now what address to
send it to. If you choose to have a refund to your card, you will have to let
us know the card information (since we don't keep credit card information
on file). This should only be done in person or over the phone. Thanks.

RETURN TO INDIA UNEXPECTEDLY. HIS FATHER WAS TAKEN
SERIOUSLY ILL. HE NEEDED TO RETURN TO INDIA TO BE WITH HIM.

DR. PENDSE WILL RETURN AS SOON AS CIRCUMSTANCES PERMIT.
WE DO NOT KNOW WHEN THAT WILL BE.

YOU WILL HAVE PRIORITY FOR AN APPOINTMENT WHEN DR.
PENDSE RETURNS. IN THE MEANTIME, YOU CAN EITHER LEAVE
YOUR PAYMENT AS A DEPOSIT FOR YOUR FUTURE APPOINTMENT,
USE IT AS A CREDIT FOR SERVICES IN THIS OFFICE OR HAVE IT
REFUNDED.

PLEASE CONFIRM BY REPLY THAT YOU RECEIVED THIS MESSAGE.
THIS WILL REDUCE THE TIME WE HAVE TO SPEND ON THE PHONE
MAKING SURE EVERYONE IS CONTACTED.
