Dear Brothers and Sisters,

If you have donated this year, or participate in the church or community in some way, we ask that you take a moment to fill out this very brief survey.

Even if you have filled this out in previous years, and your information has not changed, please fill this out again. It helps a great deal on the admin side of things.

If you are new, we do not yet have your birthday and we'd like to add it to the list if that is okay with you.

If you have donated over $250 and thus will receive a tax letter, we need your address, and it's easier to get it again now than to update entries individually.

We also need to know if you have donated by a business name or nickname different from where your name is stated elsewhere in our communications with you.
