I signed up with a form company to help me  keep better records. I set up two forms so far, a prayer request and a team shift member reference. I'd like to have you all take a look and see if there's anything which can be added.

 

How these forms are used: when you fill it out and hit submit, it gets sent to the email or emails of choice. For example, we can start putting the names of the peopleneeding healings in the light in the team shift meetings. When someone shows interest in becoming part of team shift, they can fillout the form and it will be sent to the liason and myself.... There's also a file stored that keeps everyone who filled out a specific form in a folder on the web so anyone can access)

 

I see this as being a huge help in keeping track of new people..........................
