Since you've purchased the Peak Package I wanted to remind you of a few things:

1.  You already have the Technology-Assisted Counseling/Telehealth Consent in your package.  

2. We have updated the Superbill to reflect the Place Of Service (POS) and modifiers due to the fast changes in telehealth.  We did a great FB Live today all about insurance, telehealth and billing.  In case you missed it CLICK HERE.  

3.  If you are offering a Sliding Fee Scale during this time we recommend using the Sliding Fee Scale Form. 

We have attached all the documents for you for your convenience.
