I still remember a particularly distressing evening, back in the early years of my practice…

I was working late (as I often did back then) and I had this “uh-oh” moment.

My new client just left and I was reviewing the intake forms they completed.

My stomach dropped when I noticed a huge mistake. 

There was a huge section of important information that was missing, because I naively accepted a colleague's hand me down paperwork. Sound familiar? 

The issue was bigger than I perceived because I had been using this template for WEEKS!

Fair to say - I did NOT have a restful night’s sleep...
