Dear Friend ,
The Federal guidelines for HIPAA (The Health Insurance Portability and Accountability Act) require a signed and dated BAA (Business Associate Agreement). While reviewing our paperwork, it has come to our attention that we do not poses a current BAA for your organization. Typically this form is provided by the covered entity (your organization) to the business associate (Davis Medical.) If your organization is unable to provide a form, please use the form we have enclosed. HIPAA requires there be an expiration date on the BAA. For your convenience, we have set an expiration date of 5 years.
Additionally, HIPAA also requires Davis Medical to provide information to you our client regarding our storage practice of the protected health information received from, and or created by your organization. Please read, sign and return the enclosed form “HIPAA compliant storage notification”.
