STC 1 CVS 14Sep18 65 put (CVS180914P65) for a debit of $.44  (buying a vertical) 

BTO 1 CVS 14Sep18 75 call (CVS180914C75)
STC 1 CVS 14Sep18 72 call (CVS180914C72) for a credit of $1.05  (selling a vertical) 

Be prepared to change these price limits by $.05 or more in order to get an execution.
